To Michael Rank (Mac usage):

Yes, I should have written "U.S. Extended" keyboard driver instead of 
"US-English" keyboard ... there are several ENGLISH input methods 
available, and "Extended" is in this case equivalent to Unicode. (I 
already took the freedom to correct this in the web version.)
Go to "System Preferences" --> "Language & Text" --> "Input Sources" 
and replace "U.S." with "U.S. Extended" (or just add it).
