That's an easy one ... except for his date of death. Please try to 
search in some standard Japanese who's who regarding that period.

Hiraishi Ujindo 平石氏人 (1864－?)

Also, if you consult modern Chinese histories (by Chinese and Korean 
Chinese historians) on Northeast China (Tongbuk) you'll find more 
references to him -- but no date of death given there either.
